Rules of the Small Claims Court Forms
Electronic versions of forms under the Rules of the Small Claims Court, O. Reg. 258/98 , are available in the table below, in a viewable (Adobe PDF) and fillable (Microsoft Word) format. The fillable format can be completed on-line and saved to your computer for future reference.
These forms cannot be filed electronically; they must be printed and filed in hardcopy at the court office.
